Sandy Alcantara spent the year rehabbing from Tommy John surgery undergone in October 2023. The timing of the procedure limited the right-hander to bullpen sessions, though he began to face live hitters in mid-September. All the progress reports were encouraging, but when he takes the hill in the Grapefruit League, it will be more than16 months since Alcantara threw a ball in anger. Typically, the exercises involved in Tommy John rehab strengthen the area, allowing the pitcher to match, and often add, velocity. The concern is always command and control, which were Alcantara's main assets before the injury. Even with a 98-mph fastball, Alcantara's strikeout rate was above average, but not elite. Workload will also be an issue. Rostering Alcantara is a matter of risk tolerance and level of reliance. It's best to treat him as a luxury item and not a staff fixture.

    Alcantara is expected to build up to 80-to-90 pitches while pitching in a minor-league game Friday, Chuck King of MLB.com reports. It should be Alcantara's last outing of the spring before he takes mound for the Marlins on Opening Day. The right-hander has been impressive in camp in his return from Tommy John surgery, posting a 0.00 ERA and 10:3 K:BB over 12.1 Grapefruit League innings, and if he shows he's back to something close to his Cy Young form, it might serve to increase the trade package Miami can get for him this summer.

    Alcantara gave up two unearned runs on four hits over 2.2 innings in Monday's split-squad game against the Nationals. He struck out three without walking a batter. The right-hander tossed 57 pitches (33 strikes) as he continues building up ahead of an Opening Day assignment for the Marlins. Alcantara missed the entire 2024 campaign while recovering from Tommy John surgery, but he worked over 180 innings each of the prior three seasons, and manager Clayton McCullough indicated over the weekend that Miami's ace would not face a hard innings limit in 2025. Alcantara hasn't done anything this spring to suggest he needs one, posting a 0.00 ERA and 8:2 K:BB over 8.1 innings.

    Marlins manager Clayton McCullough said Sunday on the Marlins Radio Network that "no innings limit" has been set for Alcantara this season. The skipper backed up comments from Alcantara himself, who said Saturday that the team hadn't informed him of any innings restrictions. That contradicts an earlier report from Jeff Passan of ESPN.com. The right-hander missed all of last season while rehabbing from Tommy John surgery, but he had a relatively normal offseason and is poised to start Opening Day for the Marlins.

    Alcantara threw two pitches that reached 100 mph during Friday's Grapefruit League start against Atlanta, Christina De Nicola of MLB.com reports. The 2022 NL Cy Young Award winner tossed 30 pitches in total over two scoreless innings, giving up two singles and striking out three without walking a batter. "Everything feels good with my arm, [it] was good out there. I just want to show people that I'm healthy and I can throw hard," Alcantara said after the outing. The 29-year-old righty hasn't pitched in an MLB game since September 2023, missing all of 2024 due to Tommy John surgery, but he appears fully recovered and on track to take the bump for the Marlins on Opening Day. The Tigers' Tarik Skubal, who won the AL Cy Young in 2024, is the only other starting pitcher to light up a radar gun for triple digits so far this spring.

Alcantara is headed to Miami to be evaluated this weekend by the Marlins' medical staff, and it's a safe bet that he is not going to be able to return before the end of the regular season.
